Holiday Gift Program

Families are eligible to receive gifts if they are currently experiencing homelessness or at risk of becoming homeless (for example, food insecurity, low income, recent job loss, etc.). Below, please find important information about our Holiday Gift Program:

  • This form can be completed by families requesting for their own children or by a case manager, public health nurse, etc. on behalf of a family.
  • Submitting this form does not guarantee you will receive holiday gifts. Availability will be dependent on the donations we receive.
  • Please ensure only one form is submitted per family. Multiple submissions will not increase your likelihood of receiving gifts.
  • If you are requesting for a family with multiple children, only one form needs to be completed. You will be able to request for all children in one submission.
  • Gifts can be requested for children between the ages of newborn – 17 years. If a parent is pregnant, the baby’s due date must be on or before 12/31/23. We are unable to provide gifts for children born in the new year.
  • We will contact you if we can fulfill your request. Once contacted, you will have 7 business days to pick up the gifts (the day you are contacted does not count toward the 7 days).
  • All information gathered on this form will be kept confidential and will not be shared.
