Wellspring EAP provides Employee Assistance Program Services to employers headquartered in the Pacific Northwest with offices throughout the country. Serving over 190,000 employees and their families, Wellspring is committed to providing meaningful solutions to work and life challenges that optimize well-being and productivity.

If you are a therapist interested in working with Wellspring EAP, and meet our minimum credentialing requirements, you are invited to submit your information using the form below to be considered for affiliation. If you are a representative from a facility seeking affiliation for clinicians, please call 1-866-470-2301 (select option 1).

Minimum Credentialing Requirements:

  • At a minimum you have a Masters Degree and fully licensed to meet your State Regulations for practice.
  • Maintain a professional office location.
  • Substance Abuse Counselors must be Certified or Licensed to meet state regulations for practice.
  • Maintain a minimum of one million dollars malpractice insurance per incident, three million aggregate.
  • You have not been subject to disciplinary actions, nor been convicted of a felony.
  • We do not accept any interns or other unlicensed providers.
  • We do not have clients that we can offer to associate-level providers outside Washington state due to our current contractual obligations with the employers we work with. We apologize for any inconvenience.
  • For associate level providers residing in WA who would like to join our EAP network, we have additional onboarding requirements. Before completing the online provider application, please reach out to us for more details at intake@wellspringeap.org or call us 1-866-470-2301.

Our general response time for provider inquiries is anywhere from 3-5 business days. If you would like to follow up beyond that time frame, please call 1-866-470-2301 (option 1) between the hours of 9:00 AM – 4:00 PM Pacific Time. There is no need to resubmit the form.
